Cuidado de enfermería ante el maltrato en el adulto mayor que presenta síndrome de demencia: revisión narrativa de la literatura

Abstract

Un buen reconocimiento de los diferentes generadores de maltrato del adulto mayor que presenta síndrome de demencia y al identificar sus consecuencias, favorecen con el propósito de determinar el manejo oportuno y las principales características del cuidado de enfermería con base a la teoría del cuidado humanizado de Watson según a las necesidades de cada persona en su entorno, lo cual permiten promover y proteger la salud del adulto mayor integrando los diferentes campos del hacer enfermero mediante el análisis de estudios realizados a nivel mundial.

El objetivo de la investigación fue describir el cuidado de enfermería ante el maltrato en el adulto mayor que presenta síndrome de demencia.

El desarrollo del proceso investigativo de tipo Revisión Narrativa de la Literatura; se definió y se estructuró la pregunta PICO de investigación aplicando la estrategia Anciano y Maltrato al Anciano y Trastornos Neurocognitivos mayores (o demencia), en idiomas español, inglés y en portugués, planteada de todos los diseños secundarios: Como artículos de estudios secundarios, revisiones sistemáticas, guías de práctica clínica, estudios epidemiológicos (observaciones y experimentales) y estudios cualitativos, cuantitativos y revisiones narrativas de la literatura.) y que estén en los índices LILACS, PUBMED, REDALYC, SCIELO, SCENCEDIRECT y GOOGLE SCHOLAR, de los últimos 20 años. Se incluyeron 50 artículos, los cuales cumplían los requisitos de búsqueda. Por tanto, tenían en común temas como los diferentes generadores de maltrato, consecuencias de maltrato, características del cuidado de enfermería en la prevención y el manejo oportuno del maltrato en un adulto mayor que presenta síndrome de demencia. La mayoría de los estudios aportaban información importante para la construcción de los capítulos de cada apartado y así identificando la importancia de la intervención y el papel de enfermería ante esta problemática de salud pública como lo es el maltrato a dicha población con el fin de garantizar mayor calidad de vida y bienestar supliendo sus necesidades básicas y fundamentales. Como conclusión general; durante la revisión narrativa de la literatura se pudo describir aspectos relacionados al cuidado de enfermería ante el maltrato en el adulto mayor que presenta sindrome de demencia, el cual se presenta en todas las clases sociales y a cualquier nivel socioeconómico. A la vez se puede dar tanto en la comunidad como en instituciones residenciales ya sean públicas o privadas. Pero hay que tener en cuenta que se dan diferentes tipos de maltratos y que un mismo anciano puede ser víctima de varios al mismo tiempo, teniendo en cuenta que en los maltratos están más implicados por los propios familiares y cuidadores reflejado por la sobrecarga del mismo, el desconocimiento y /o la negación del problema por parte de los profesionales de salud afectan a la prevención, detección e intervención.

A good recognition of the different generators of mistreatment of older adults with dementia syndrome and the identification of its consequences, favor the purpose of determining the timely management and the main characteristics of nursing care based on Watson's theory of humanized care according to the needs of each person in their environment, which allows promoting and protecting the health of older adults by integrating the different fields of nursing through the analysis of studies conducted worldwide.

The objective of the research was to describe nursing care in the face of mistreatment in older adults with dementia syndrome.

The development of the research process of Narrative Literature Review type; the PICO research question was defined and structured applying the strategy Elderly and Elder Abuse and Major Neurocognitive Disorders (or dementia), in Spanish, English and Portuguese languages, raised from all secondary designs: such as articles of secondary studies, systematic reviews, clinical practice guidelines, epidemiological studies (observational and experimental) and qualitative, quantitative studies and narrative literature reviews. ) and that are in the LILACS, PUBMED, REDALYC, SCIELO, SCENCEDIRECT and GOOGLE SCHOLAR indexes, from the last 20 years. Fifty articles were included, which met the search requirements. Therefore, they had in common topics such as the different generators of mistreatment, consequences of mistreatment, characteristics of nursing care in the prevention and timely management of mistreatment in an older adult with dementia syndrome. Most of the studies provided important information for the construction of the chapters of each section and thus identifying the importance of intervention and the role of nursing in this public health problem such as the abuse of this population in order to ensure better quality of life and well-being by supplying their basic and fundamental needs.

As a general conclusion, during the narrative review of the literature it was possible to describe aspects related to nursing care in the elderly with dementia syndrome, which occurs in all social classes and at any socioeconomic level. At the same time, it can occur in the community as well as in residential institutions, whether public or private. But it should be noted that there are different types of abuse and that the same elderly person can be a victim of several at the same time, taking into account that abuse is more involved by family members and caregivers themselves reflected by the overload of the same, ignorance and / or denial of the problem by health professionals affect the prevention, detection and intervention.
